Payment for services is required at the time of your visit. We accept payment via checks and all major credit cards. If you plan to have a third party cover your fees, please make arrangements with our staff in advance. We will provide you with all the necessary documentation to submit a claim to your insurance carrier for reimbursement. The amount you will be reimbursed depends on the type of insurance plan you have and your carrier's policies. We encourage you to contact the "800" number on the back of your insurance card to know the level at which your company will reimburse you for services of an "out of network" provider. If your insurance carrier reimburses you for our services, we can speak with an insurance representative to ensure proper claim submission. Following your visit, we will provide you with a Superbill, which you can fax to your carrier's claims department. Please note that our services are not reimbursable by Medicare as we have "opted out" of Medicare.

Dr. Schulte's practice is telemedicine-based. He is licensed in Arizona, California, and Massachusetts and prescribes in most other states.
New patients unknown to Dr. Schulte and some new patients known to Dr. Schulte but with whom he has not had adequate contact to provide sufficient background to create a framework for further treatment.
Generally, these are new patients with whom Dr. Schulte has already had multiple contacts through a family member and on whom he has sufficient background to lay the foundation for further individual work.
